A night shift worker died after he became trapped in a machine at a factory in Berkshire, a court has heard. Mitesh Prashar was working an automatic inspection machine at the site in Eastbourne Road on the Slough Trading Estate for Duco International Limited. The 24-year-old was found with his left arm, shoulder, head and torso trapped by the machine, Reading Crown Court heard. An investigation by the Health and Executive following the incident found it had failed to provide a safe system of work which had led to the worker's death. It found the Manchester-based company had not considered the risks linked with using the machine and had not provided proper information, instruction or training to employees on its use. The court also heard how there were no guards in the machine to stop workers from accessing dangerous parts other machine. Duco International Limited, of Varn House, Northbank Industrial Park, Irlam, Manchester, was fined £200,000 and ordered to pay costs of £43, 352 after admitting breaching health and safety laws.
